 Position Overview:

Under the direction of the Asst. Director of Community Programs, this position provides support to Jason Hyatt Center.  The Student Intern will provide support services, both psychological and social, to chronically mentally disabled and/or co-occurring, young adult clients, who are making the transition from institutional care to an independent living setting in accordance with the treatment plan. A key area of focus is to connect with young people who are facing significant barriers in their lives. This role requires significant relationship building/engaging skills, the ability to meet young people where they are, and to support them to identify goals in an effort to make positive changes in their lives.

Essential Functions:

• Under the guidance of Jason Hyatt Center employees, provide daily oversight to all residents involved in the program
• Collaborate with JHC team regarding strategies in supporting young people to achieve their goals
• Report all crises to the Asst. Director of Community Programs, or designee, in a timely manner
• Accompany clients to the SCSI Social Rehabilitation programs as scheduled
• Observe while employees supervise residents self-administering medications as scheduled
• Assist with completing daily documentation as required by the program, including navigating within and completing entries in the electronic medical record
• Assist with facilitating scheduled activities and groups based on life skills assessments
• Complete apartment safety checks to ensure residents are maintaining cleanliness/upkeep of the apartment
• Assist residents on developing daily living skills such as cooking, cleaning, shopping, transportation, budgeting, personal hygiene, sex education, and other areas.
• Report all changes in behavior or mental state to appropriate team members (i.e. JHC employees, case manager, Asst. Director of Community Programs, etc.).
• Assist employees with managing their caseload daily, which includes, but is not limited to Positive Rewards, activities, mentoring, d/c planning notes, and other ADL skills.
• Provide direct feedback to Asst. Director of Community Programs, or JHC employee designee, as required, to ensure that proper treatment and support is being provided. 
• Under direction of Assistant Director of Community Programs, or designee, ensure individual support plan is implemented in such areas as: daily living skills, education/employment, social supports, health, mental health, addictions, housing, physical health, harm reduction, leisure and recreation, etc.
• File all paperwork as scheduled.
• Maintain cleanliness of the office daily which includes but is not limited to vacuuming, dusting, sweeping, mopping, disinfecting, and removing trash.
• Build rapport with residents and AXS participants
• Assist JHC employees with connecting young people to community resources
• Work collaboratively with community agencies
• Advocate for all young people involved in the AXS program

Physical Requirements:

While performing the duties of this job, the employee is regularly required to sit, use hands to finger, handle or feel objects, tools, or controls, and climb stairs. The employee frequently is required to walk, talk and/or hear; Speaking and hearing ability sufficient to communicate effectively by phone or in person at normal volumes; Vision adequate to read correspondence, computer screen, forms, etc. The employee is occasionally required to stand and climb or balance. The employee must occasionally lift and/or move up to twenty-five (25) lbs. Specific vision abilities required by this position include the ability to adjust focus.

Qualifications:

High School diploma or equivalent required; current student in an accredited college program, with requirements in collaboration with a course syllabus; proficient in the Microsoft Office suite; excellent verbal and written communication; high ethical standard and practice.
