Function(s):

Assists HCBS and/or ER Department with care management and advocacy for the HCBS/ER Division.

JOB RESPONSIBILITIES: Care Management Intern

Assists care managers with providing care management for managed care enrollees in the LTSS division

  1. Evaluate assessment and care management data to determine ongoing program eligibility, care needs and service options.
  2. Understand how to make Level of Care determinations for consumers as part of the initial, annual and event-based (if applicable) assessment process.
  3. Monitor and evaluate the care plan, service quality and program eligibility.
  4. Collaborate with the Department of Job and Family Services to establish and/or maintain Medicaid eligibility.
  5. Assist with completing the enrollment process of eligible consumers into the program.
  6. Provide education and support to consumers and their caregivers through in-person visits or other communication.
  7. Understand how to develop and implement an individualized care plan for all consumers.
  8. Advocate on behalf of consumer and/or caregiver/family and assist individuals in securing appropriate services and care settings.
  9. Assist with dis-enrolling ineligible consumers from the program.
  10. Apply judgement and perform clinical practice based upon education, experience and professional practice standards.
  11. Participate in and contribute towards accomplishment of divisional and Agency dashboard goals.
  12. Perform data entry of enrollee information into the appropriate documentation systems.
  13. Handle incoming enrollee, provider, and other communication requests as needed for Care Managers/Waiver Service Coordinators.

Qualifications

  • Maintain and submit timely and accurate documentation of activities in compliance with standards determined by field instructor.
  • Complete six (6) hours of Continuing Education Training annually.
  • Exemplify the Agency 3C’s corporate culture in all activities and interactions with staff, members, consumers, stakeholders as well as the community at large.
  • Attend All Staff meetings and Unit Meetings as able.
  • Meet all requirements as established and identified in the learning contract.
