Internship Title: Pathway’s to Success Social Work Intern

Agency Overview: Community Counseling Centers of Chicago (C4), is a behavioral health advocate and social service provider, offering a quality, comprehensive customer-oriented services tailored to the diversity of its consumers.

Internship Description: As a Social Work Intern at C4 Pathway’s to Success program, you will have the opportunity to gain hands-on experience in a dynamic and supportive environment under the guidance of experienced social work professionals. This internship is designed to provide you with exposure to various aspects of social work practice, including case management, client advocacy, program development, and community outreach.

Key Responsibilities:

1. Care Coordination/Case Management: Assist social workers in conducting client assessments, developing individualized service plans, and coordinating access to resources such as therapy, education, community supports, housing, healthcare, and financial assistance.

2. Client Support: Provide direct support to clients and families through child and family team meetings, home visits, crisis intervention, and emotional support. Help clients navigate complex systems and advocate for their rights and needs.

3. Program Assistance: Assist in the development, implementation, and facilitation of appropriate supportive groups and services. Shadowing Care Coordinators during designated family meetings. Contribute ideas for program improvement and expansion during weekly in-person staff meetings.

4. Community Outreach: Identifying community supports based on client’sstrengths and needs and assist clients with access and enrollment.

5. Administrative Tasks: Complete administrative tasks as needed, such as maintaining client records, scheduling appointments, and documenting client interactions in accordance with agency policies and procedures.
